United Way youth leadership Award

United Way of Coles County is excited to roll out the United Way Middle School Community Service Award. Community members can nominate a 6th, 7th, or 8th grade student who has displayed the criteria below. Please complete the form below, and have the student write a summary on why they should receive the community service award. 


Documents are due by APRIL 22. One form must be completed for each student nominated. United Way will choose one male and female representative from each grade level to be recognized.
